Physical Therapist

The Physical Therapist is responsible for providing direct patient care, including patient evaluations, consultations, and treatments aimed at improving physical function and mobility. This role involves conducting individualized assessments and designing customized, goal-directed plans to help patients achieve maximum physical performance. Responsibilities:

• Perform comprehensive patient evaluations to assess physical abilities and limitations.
• Design and implement individualized treatment plans tailored to patient needs and goals.
• Provide direct therapeutic interventions to improve patient mobility and function.
• Consult with patients and interdisciplinary team members regarding treatment progress and adjustments.
• Document patient assessments, treatment plans, and progress notes accurately and timely.
• Educate patients and families on exercises, mobility techniques, and injury prevention strategies.

• Licensed Physical Therapist in the state of practice.
• Knowledge of physical therapy assessment and treatment techniques.
• Strong clinical evaluation and patient care skills.
• Excellent communication and interpersonal skills.
• Ability to develop individualized treatment plans.

• Experience working in outpatient or clinical rehabilitation settings.
• Familiarity with electronic medical records and documentation systems.
• Additional certifications in specialized physical therapy areas (e.g., orthopedics, geriatrics).

• Work involves direct patient contact and physical activity.
• May require collaboration with interdisciplinary healthcare teams.

• Work schedule consists of 4 days per week, 10 hours per day.
• Day shifts with consistent hours.
