Each home health risk assessment typically takes approximately 45 minutes and may involve annual health evaluations or post-discharge visits.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule for maximum convenience
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage members
• Review patients prior and current medical history, conditions, and medication usage
• Educate members on their health conditions and proper medication management
• Identify gaps in care and recommend solutions to promote optimal health outcomes
• No direct treatment planning or medication prescribing required

Qualifications:

• Active Nurse Practitioner License in Illinois
• Current BLS certification (CPR)
• Board Certification through AANP or ANCC
• Minimum of 6 months experience in nurse practitioner role preferred
• MSN degree from an accredited nursing program
