Key Responsibilities\n

• Assist primary care providers in identifying, addressing, and managing mental health and psychosocial issues within a primary care setting.

• Conduct thorough patient intakes, including diagnostic and functional evaluations, and develop appropriate treatment goals in consultation with the primary care team.
\n
• Deliver brief, focused therapeutic interventions for clients seeking mental health support, adapting techniques to suit individual needs.
\n
• Evaluate patients’ requirements and provide guidance on whether continued care should occur in the primary care setting or if a referral is necessary to a specialized mental health facility.
\n
• Initiate follow-up care to monitor patient progress, adjusting treatment plans as necessary to ensure optimal outcomes.
\n
• Develop and implement relapse prevention plans to assist patients in maintaining stable mental health.
\n
• Detect and formulate intervention strategies for 'at-risk' clients to prevent deterioration in mental well-being.
\n
• Coordinate behavioral health services involving communication and collaboration not only among our primary care team but also with external service providers when necessary.
\n
• Maintain comprehensive documentation in treatment charts to track patient progress and inform ongoing care strategies.
\n
• Educate patients about self-management techniques to empower them in their recovery journey.

Qualifications\n

• A Master’s Degree in Clinical Counseling is required.

• Current LPC (Licensed Professional Counselor) License is mandatory.

• Comprehensive knowledge of therapeutic assessment, treatment planning, developmental psychopathology, and evidence-based practices in individual and group therapy.

• Strong understanding of DSM-IV diagnoses and a variety of psychotherapeutic techniques.

• Ability to maintain effective relationships with clients, fellow professionals, and community resources.

• Ability to express ideas clearly and implement sound clinical judgment throughout patient interactions.

• Proficient clinical documentation skills to maintain patient records accurately.

Preferred Experience\n

• Experience in an interdisciplinary healthcare team setting is desirable.

Compensation and Benefits\n

The salary for this position ranges from USD 87,000 to 105,000 annually, commensurate with experience and qualifications. In addition to competitive pay, we offer a generous sign-on bonus to our new counselors.
