Position Summary and Qualifications

At our organization, we treat each other like family. As a Case Manager in our Rehabilitation program, you will play a crucial role in assisting patients and their families throughout the healing journey, ensuring that each individual receives high-quality, personalized care that fosters recovery, independence, and hope.

Your key responsibilities will include:

• Assisting patients and families during program orientation, explaining rehabilitation principles, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinating educational sessions for patients, families, and caregivers to promote active participation in treatment planning, goal-setting discussions, and family meetings.
• Creating and executing tailored treatment plans that reflect the patient’s strengths, needs, and personal recovery objectives.
• Conducting psychosocial assessments and discharge planning evaluations, accurately documenting findings and communicating needs with the care team.
• Working closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure cohesive, patient-centered care.
• Upholding our organization's values of compassion, respect, and excellence in every interaction with patients and families.

Required Qualifications:

• Registered Nurse (RN) license.
• Associate Degree in Nursing.
