Key Responsibilities\n

• Conduct comprehensive evaluations and assessments for children aged zero to three, identifying developmental delays and specific needs.

• Actively participate in the creation and execution of Individualized Family Service Plans (IFSP), working closely with families to outline and achieve developmental goals.
\n
• Develop rapport with children, families, and interdisciplinary team members to foster a nurturing and supportive learning environment.
\n
• Provide actionable support and resources to families, equipping them with strategies that utilize evidence-based practices within Natural Learning Environment frameworks.

• Monitor and track developmental outcomes as part of a committed team, ensuring the highest standards of care and compliance with all federal and state regulations.

• Utilize a coaching model to deliver family-centered care, promoting engagement and participation in the child's developmental journey.
\n
• Maintain meticulous documentation of assessments, plans, and interactions while adhering to confidentiality and ethical standards.

Patient Population / Client Focus\n

Your primary focus will be on infants, toddlers, and their families, particularly those from underrepresented communities facing challenges in accessing early intervention services. By providing targeted support to mitigate developmental delays, you will play an essential role in enhancing not just the children’s outcomes but also their families’ ability to advocate for their developmental needs.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work.

• Current and valid LCSW license issued by the Department of Public Health.

• A valid driver’s license is required to facilitate travel for in-home visits when necessary.

• Proficient in adapting therapeutic approaches to align with the individual needs of each child and family.

Preferred Experience\n

• At least one year of direct experience working with children in an early intervention context.
