Key Responsibilities\n

• Engage with clients both via phone and in-person to assess their needs and gather pertinent information related to their situations.

• Conduct thorough evaluations to create personalized treatment plans that address each client's emotional and mental health requirements.

• Provide crisis intervention services, employing your crisis management skills to support clients in distress and facilitate immediate assistance.
\n
• Maintain accurate documentation and case records, preparing necessary reports that outline client progress and interventions for ongoing evaluation.
\n
• Facilitate referrals to appropriate community resources, collaborating with other agencies to ensure clients receive holistic, well-rounded care.
\n
• Participate in regular training sessions, supervision, and team meetings, contributing to a culture of continuous improvement and professional growth.

• Recognize and report signs of child abuse, ensuring prompt action and adherence to statutory reporting requirements.

• Encourage client participation in beneficial activities to enhance their coping strategies and support networks.

Patient Population / Client Focus\n

In this role, you will be focusing on a highly sensitive patient population that includes individuals and families dealing with severe emotional challenges stemming from life-threatening illnesses, neglect, and abusive situations. Working primarily with clients in hospice care, you will facilitate discussions surrounding end-of-life decisions, coping mechanisms, and family dynamics to ensure that each client feels supported and understood during these critical times.

Qualifications\n

• A Master’s degree in social work, psychology, or a related field is required, with a thorough understanding of clinical practices relevant to social work.

• Current licensure as a Clinical Social Worker (LCSW) is mandatory.

• Exceptional interpersonal skills, with the ability to empathize with clients and remain composed in challenging situations.

• Strong analytical skills to assess client needs effectively, coupled with the ability to provide appropriate interventions based on their unique circumstances.

• Familiarity with case management software and proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) for effective documentation and communication.

• Possession of a valid driver’s license, enabling some in-person client meetings when necessary.

Compensation and Benefits\n

This role offers a competitive salary ranging from $90,000 to $100,000 annually, along with opportunities for monthly productivity bonuses. In addition, we provide:
