Work Environment

This role is located onsite, specifically in home care settings where direct patient interaction takes place. The work schedule generally follows a Monday, Wednesday, and Friday routine from 8:00 a.m. to 5:00 p.m., but we are open to flexible scheduling to accommodate the needs of our professionals. Key Responsibilities

• Provide comprehensive primary care services tailored to each patient's needs, ensuring all patient visits are thorough and empathetic.
• Conduct onsite health risk assessments to proactively identify potential health risks, paving the way for preventive interventions.
• Deliver wellness education tailored to the individual patient, focusing on preventive measures and encouraging healthy lifestyle choices.
• Support individuals in making positive lifestyle changes, offering guidance and resources to enhance their overall well-being.
• Assist patients in managing chronic conditions effectively, developing individualized care plans that address their unique challenges and goals.
• Oversee and ensure that quality care is provided by the medical staff at Southern Healthcare Center, aiding in the development of standardized best practices.
• Foster a strong collaborative relationship with the center's multidisciplinary team to ensure successful health outcomes for patients.

Patient Population / Client Focus

In this role, you will serve a diverse patient population that encompasses families, adults, and individuals dealing with various health issues. Our commitment to preventive medicine allows for a proactive approach to healthcare, aimed at managing chronic conditions, implementing wellness strategies, and improving the overall health of our community. As we provide telehealth services, you will also have the opportunity to consult with patients remotely, ensuring care reaches those who may have difficulties attending in-person visits.

Qualifications

• Master’s Degree in Nursing from an accredited program.
• A current and valid Family Nurse Practitioner license in Florida.
• Knowledgeable about and compliant with the operational standards set forth by Southern Healthcare Center.
• Ability to maintain professional and cordial relationships with clients and coworkers, contributing to a positive work environment.
• Experience utilizing telehealth technology to consult with and provide care to patients effectively.
• Demonstrable ability to work efficiently in a fast-paced, autonomous environment, managing multiple patient needs and prioritizing tasks effectively.

Preferred Experience

• Proven track record as a Telehealth Clinician, administrating remote patient care with positive outcomes.
• Previous experience in a home care or outpatient setting, particularly in managing chronic conditions and preventive care strategies.

Compensation and Benefits

The Family Nurse Practitioner position at Southern Healthcare Center comes with a competitive annual salary range of $120,000 – $130,000, commensurate with experience. In addition to competitive pay, we offer a robust benefits package that includes:
