Key Responsibilities\n

• Conduct thorough and holistic mental health assessments for hospice patients, considering their medical history, psychological state, and emotional needs.

• Work closely with patients and their families to develop personalized care plans that address emotional and psychological challenges, facilitate coping strategies, and promote overall well-being.

• Implement evidence-based therapeutic interventions, including individual and family therapy sessions tailored to the unique circumstances of hospice care, such as grief and loss counseling.

• Remain available for crisis intervention, providing support during moments of emotional distress or urgency, and ensuring appropriate referrals for additional levels of care if necessary.

• Engage in psychoeducation with both patients and their families, helping them understand the emotional processes associated with end-of-life care and available resources.

• Document all interactions, treatment plans, and progress notes meticulously in compliance with legal requirements and ethical guidelines, using electronic health record (EHR) systems.

• Participate in multidisciplinary team meetings to ensure cohesive and coordinated care, enhancing treatment plans through collaborative feedback.

• Stay up-to-date with best practices, regulations, and trends in hospice care and mental health support.

Patient Population / Client Focus\n

The ideal candidate will work primarily with terminally ill patients and their families, providing mental health support at a time when emotional needs are often heightened. Qualifications\n

• Must possess a Master’s degree in Social Work from an accredited institution.

• Active and valid LCSW licensure in California is required.

Compensation and Benefits\n
