Key Responsibilities\n

• Conduct thorough psychosocial assessments for each client, crafting individualized treatment plans aimed at addressing their specific emotional and psychological needs.

• Utilize evidence-based interventions such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT) to promote client well-being, resilience, and mental health stability.

• Meticulously maintain clinical documentation and records, ensuring compliance with HIPAA regulations and organizational standards for confidentiality and ethical practice.

• Collaborate with a multidisciplinary treatment team, including nurses, hospice staff, and other social workers, to coordinate comprehensive care plans that address the whole person.

Patient Population / Client Focus\n

Your clientele will range from individuals suffering from chronic illnesses to those seeking support for emotional distress related to end-of-life care. This setting requires a compassionate approach as you'll be guiding clients and their families through some of life’s most challenging moments. You will advocate for their needs, helping them navigate through complex emotions associated with grief, loss, and terminal diagnoses.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• A current and valid Licensed Clinical Social Worker (LCSW) license in the State of Oregon, free from any disciplinary actions.

• Proven experience in clinical social work, including familiarity with mental health assessment, diagnosis, and tailored treatment planning.

• Excellent interpersonal and communication skills, capable of establishing rapport with a diverse client base and collaborating effectively with an interdisciplinary team.

Compensation and Benefits\n

The salary for this full-time position is competitive and ranges from $52,000 to $70,000 annually , commensurate with education and experience.
