Responsibilities

• Administer prescribed medications and treatments per orders and facility policies
• Perform ongoing resident assessments, monitor status, and document findings
• Report changes to the supervising RN and contribute to care plan updates as authorized
• Provide direct patient care, including activities of daily living, hygiene, and mobility assistance
• Manage wound care and dressing changes; monitor for signs of infection
• Maintain safety, infection control, and falls prevention; respond to call bells promptly
• Collaborate with licensed staff and the interdisciplinary team to deliver consistent care

Requirements

• Active LPN license to practice in New Mexico
• BLS/CPR certification
• Ability to work the night shift (18:00–06:30) and meet guaranteed hours
• Prior long-term care or skilled nursing experience preferred
• Proficiency with charting in an electronic medical record system
• Diploma or certificate from an approved practical nursing program
