Occupational Therapist - Home Health

• Mission-driven, supportive culture Requirements What You'll Do • Perform OT evaluations and ongoing assessments, including OASIS as required
• Develop, implement, and revise individualized plans of care
• Provide therapeutic interventions for ADLs, ROM, strength, coordination, and adaptive equipment
• Educate patients, families, and caregivers
• Maintain timely and accurate documentation
• Collaborate with physicians and interdisciplinary teams
• Supervise COTAs and aides as applicable
• Follow infection control and safety standards What We're Looking For Required:
• Bachelor's degree in occupational therapy
• Current New Mexico OT license
• Valid driver's license, reliable transportation, and auto insurance
• Current CPR certification Preferred:
• 1+ year OT experience
• Home health experience
• Homecare Homebase proficiency
• Strong communication skills Physical & Work Environment Requirements • Ability to lift, bend, push, pull, and stand/walk for extended periods
• Manual dexterity and hand-eye coordination
