The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of Option Care, pharmacy practice professional standards, and applicable regulatory entities. This is an entry level position into the practice of clinical pharmacy in the alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals and educates patients, caregivers, staff and other professionals regarding appropriate medication use. Job Description The Clinical Pharmacist will manage and execute comprehensive pharmacy care for home infusion patients, ensuring medication safety, efficacy, and compliance with all regulatory standards. Job Responsibilities (listed in order of importance and/or time spent) Determine the suitability of individual patients for home care, collecting and organizing all patient-specific information to assess medical condition and prescribed medication therapy suitable for home care services. Design, recommend, monitor and evaluate patient-specific pharmacy care plans for the pharmacotherapy of home care patients; make suitable recommendations for modifications in prescribed medication therapy or monitoring parameters. Modify pharmacy care plans based on evaluation of monitoring data and other relevant patient-specific information; apply pharmacokinetic principles and assess patient laboratory values. Provide medication‑use education to Option Care staff, other professionals, home care patients and/or their caregivers using effective patient education techniques. Ensure continuity of pharmaceutical care to and from the home and other patient‑care settings, communicating pertinent patient information and preparing/dispensing medications per policy, including calibrating equipment, preparing labels, inspecting final medication and complying with state/federal laws. Manage the use, maintenance, and troubleshooting of medication administration equipment and medication‑related equipment; participate in reporting medication‑related incidents and incident documentation. Provide concise, applicable, and timely responses to drug information requests from health care providers and patients, formulating responses based on literature analysis and maintaining CEU requirements. Manage investigational drug products according to established regulations and option care policies and effectively utilize pharmacy support personnel, prioritizing workload and ensuring accuracy. In all activities, comply with accreditation, legal, regulatory, and safety requirements. Maintain confidentiality of patient and proprietary information, observing legal and ethical guidelines for safeguarding confidentiality. Supervisory Responsibilities No supervisory responsibilities. Basic Education And/or Experience Requirements Graduate of an accredited School of Pharmacy. Current, active Pharmacy license granted by the State Board of Pharmacy. One of the following: At least one year of experience in sterile IV admixture procedures. Knowledge of non‑sterile compounding, immunizations and medication therapy management (MTM). Completion of an ASHP accredited Pharmacy Residency Program. Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy. Equivalent experience approved by the Vice President of Pharmacy and/or Senior Vice President of Clinical Services. Basic Qualifications Experience providing customer service to internal and external customers, meeting quality standards and evaluating customer satisfaction. Basic skill in Microsoft Word, including document editing, formatting, and reviewing functions. Basic skill in Microsoft Excel, including workbook creation, formatting, and data organization. Basic skill in Microsoft PowerPoint, including slide creation, editing, and presentation functions. Travel Requirements Willing to travel up to 5 % of the time for business purposes within state. Preferred Qualifications & Interests (PQIs) Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy. Experience in home care, hospital or another clean room setting. Management Requirements At least one year of experience in indirect management of team members, including assisting in development, training and assignment of work or projects to other team members. Salary and Benefits Salary to be determined by the applicant’s education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Pay Range: $102,221.94–$170,369.91.
