Pathway of Hope Case Manager: Empower Families
RadNet, Inc.
The Salvation Army seeks a skilled coordinator for the Pathway of Hope (POH) program to support families with minor children in Kansas City. You will perform outreach, intake, assessments, and case planning to help families achieve housing stability and increased resources. Responsibilities include referrals, advocacy, documentation in SIMS, and collaboration with POH staff. Some evenings and weekends may be required to meet client needs. #J-18808-Ljbffr RadNet, Inc.
