Responsibilities
- Provide direct patient care within the scope of practice for an LPN in a long-term care setting
- Monitor residents’ conditions, document vitals, administer medications under supervision, and respond to changes in status
- Assist with activities of daily living and support resident mobility, comfort, and safety
- Perform basic wound care, skin integrity checks, and infection control practices as directed
- Document care accurately, update care plans, and communicate changes to the supervising RN
- Collaborate with the interdisciplinary team, including nursing, therapy, and dietary staff, and report concerns promptly

Requirements
- Active Licensed Practical Nurse license in New Mexico (or eligibility to practice)
- Completion of an approved LPN program (diploma or degree)
- Prior long-term care experience preferred
- Ability to work night shifts (18:00–06:30) and a 3x12-hour schedule
- Strong clinical assessment, documentation, and communication skills
- Local assignment availability as indicated in the job notes
