Job Overview

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) for a local contract assignment. The ideal candidate will provide direct nursing care, administer medications and treatments, monitor resident conditions, and collaborate with the healthcare team to ensure high-quality patient outcomes.

Key Responsibilities

• Provide direct nursing care in accordance with physician orders and individualized care plans.
• Administer medications, treatments, and nursing procedures safely and accurately.
• Monitor, assess, and document resident conditions and responses to care.
• Communicate changes in resident status to physicians and healthcare team members.
• Assist with admissions, transfers, and discharge planning.
• Maintain accurate and timely clinical documentation.
• Follow infection prevention, safety, and regulatory standards.
• Support quality improvement initiatives and resident-centered care practices.
• Promote resident comfort, dignity, and overall well-being.

Requirements

• Active Licensed Practical Nurse (LPN/LVN) license required.
• Current BLS Certification preferred.
• Previous long-term care, rehabilitation, or skilled nursing experience preferred.
• Strong medication administration and clinical assessment skills.
• Excellent communication and organizational abilities.
• Ability to work effectively in a fast-paced healthcare environment.
