Physical Therapist Case Manager: Home Health

We are actively expanding into Marin and Sonoma counties. All opportunities require field visits to patients’ homes. In partnership with a case team, you will provide skilled physical therapy services to evaluate and address individual needs, enabling safe and productive home living. Responsibilities Perform home evaluations and develop individualized care plans. Work with Physical Therapy Assistants (PTAs) to implement care plans. Assist patients with mobility, movement, and regaining independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure quality of care. Evaluate and report patient progress to the attending physician. Assist with documentation and administrative tasks. Develop your own flexible work schedule and hour blocks. Targeted assignments by zip code to reduce travel time. Operate autonomously without clock‑in/out requirements. Qualifications Current unrestricted California Physical Therapist license. Current BLS/CPR certification. Valid California driver’s license. Knowledge of local community resources. Understanding of how illness/injury impacts patient development and the influence of culture on health care. Knowledge of state and federal home‑care regulatory guidelines preferred. Experience in home health care highly preferred. Experience with high‑acuity patients a plus. Experience working with an interdisciplinary team highly desired. Case management fundamentals highly desired. Self‑directed, empathetic, critical thinker with outstanding interpersonal skills. Leadership and organizational abilities.
