Key Responsibilities\n

• Conduct thorough patient intakes, focusing on diagnostic assessments and functional evaluations, and recommend treatment goals to primary care providers.

• Provide brief, targeted interventions for patients requiring mental health support.

• Actively participate as a member of the primary care team, assisting in recognizing, treating, and managing mental health and psychosocial issues.

• Monitor and follow up with patients to assess their progress and make necessary adjustments to treatment plans.
\n
• Develop and implement relapse prevention strategies that help clients maintain stability and recover from mental health challenges.
\n
• Identify at-risk patients and create comprehensive plans aimed at preventing further deterioration of their mental health status.

• Document patient progress and maintain accurate treatment records in compliance with agency policies.
\n
• Foster patient education around self-management skills and promote healthier lifestyle choices.
\n
• Provide consultation to management and team members regarding behavioral health services, focusing on program evaluation and improvements.

• Support patients in adhering to medical treatments prescribed by their primary care provider, including managing medication-related side effects.

• Uphold the Code of Ethical Conduct for Clinical Professionals and execute additional duties as assigned.

Patient Population / Client Focus\n

Your role will encompass providing care to a diverse array of patients within the community. You will work closely with individuals experiencing varying levels of mental health challenges, including anxiety, depression, trauma, and more. Qualifications\n

• A Master’s Degree in Clinical Counseling or a related field.

• Current and valid LPC (Licensed Professional Counselor) license in New Jersey.

• In-depth understanding of mental health assessments, treatment planning, and behavioral therapies.

• Proficiency in DSM-IV diagnoses and psychotherapeutic techniques.

• Strong knowledge of community resources available to address mental health and social needs.

• Excellent communication skills to effectively engage with patients and collaborate with primary care providers.

• Ability to maintain mental and physical health necessary for the demands of this role.

Preferred Experience\n

• Bi-lingual (English-Spanish) candidates are preferred but not required.

• Experience in family therapy, group therapy, or child psychotherapy is a plus.

• Familiarity with community health settings and FQHC operations is an advantage.

Compensation and Benefits\n

Thrive Hard Behavioral Counseling Center offers a competitive salary range of USD 87,000 to 105,000 per year, commensurate with experience and qualifications.
