Key Responsibilities\n

As a Licensed Professional Counselor at Mohawk Trail Counseling Center, your primary responsibilities will include:

• Conducting comprehensive patient intakes that focus on diagnostic and functional evaluations.

• Collaborating with primary care providers to identify and manage mental health and psychosocial issues, establishing effective treatment goals.

• Delivering brief, targeted interventions to patients requiring mental health services, utilizing evidence-based practices.

• Providing recommendations for referrals to specialty mental health facilities as needed.
\n
• Initiating follow-up care for patients to assess progress and make necessary adjustments to treatment plans.

• Developing relapse prevention strategies to support patients in maintaining their mental health stability.

• Monitoring the delivery of health services related to behavioral health care and coordinating with other providers, both within and outside the clinic with patient consent.

• Documenting patient progress and diagnostic information accurately in treatment charts, ensuring compliance with agency guidelines.

• Keeping primary care providers informed about patient needs and collaborating on treatment plans.

• Educationally empowering patients to engage in their own treatment and recovery through self-management skills.

• Consulting with clinic management and team members on behavioral health services and contributing to program evaluation.

• Ensuring alignment with the Code of Ethical Conduct for Clinical Professionals and performing additional duties as assigned.

Qualifications\n

The ideal candidate will possess the following qualifications:

Compensation and Benefits\n

This full-time position offers a competitive salary range of USD 87,000 to 105,000 annually.
