This position is primarily onsite at our outpatient office located in Toms River, NJ. Key Responsibilities

• Conduct comprehensive evaluations to assess patients' physical abilities, limitations, and rehabilitation goals.
• Develop and implement individualized treatment plans that use a variety of therapeutic modalities including therapeutic exercises, manual therapy, and neuromuscular re-education.
• Guide patients through exercises and physical activities while monitoring their progress and modifying treatments as necessary to maximize their recovery.
• Document all treatment sessions, including patient responses and progress towards set goals, utilizing electronic medical record (EMR) systems.
• Engage in regular reassessments and adapt treatment plans based on changes in patient condition or progress.
• Educate and train patients and their families on the importance of adherence to therapeutic exercises and recovery programs.
• Participate in continued professional development through workshops and training programs to maintain licensure and enhance expertise in specialized treatment techniques.
• Collaborate with fellow healthcare professionals in multidisciplinary team meetings to discuss patient care strategies and outcomes.

Patient Population / Client Focus

As a Physical Therapist at Constant Physical Therapy, you will primarily work with adult and geriatric patients recovering from surgeries, injuries, or chronic conditions. This includes designing rehabilitation programs for individuals recovering from orthopedic surgeries, stroke, arthritis, sports injuries, and other ailments affecting their mobility. Our approach emphasizes building strong rapport with patients and their families, ensuring that care plans align with each individual's lifestyle and recovery goals.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• Current and valid State License in Physical Therapy.
