Job description LPN Intake Coordinator Home Health and Hospice
Location: Northport, Long Island | Full-Time | Non-Profit Healthcare You Will:
Receive, evaluate, facilitate, and manage inquiries and referrals for agency services from all sources and facilitate the transition of patients from an on-site unit of a hospital or skilled nursing facility to the home or hospice house.
Develop/maintain contact with key hospital, skilled nursing, assisted living facility discharge planning services and/or management to provide ongoing updates on company's services available in a market.
Evaluate patients referred to home care/hospice care and appropriately identify 1. skilled need 2. patient safety 3. homebound status 4. patient and family ability to provide appropriate care needs for other services. Ensure effective communication and collaboration with program staff and other field sales resources via weekly meetings. Actively participate in weekly program business development meetings, bringing relevant data, reports, as well as information regarding changes within accounts and referral sources. Provide education to facility staff regarding home care and hospice. Qualifications
MSW or LPN Preferred
One year experience in acute care setting
