Work Environment

The role is primarily onsite, working within a home care setting, where you will provide therapeutic services in patients' residences. Key Responsibilities

• Deliver high-quality, patient-centered physical therapy by conducting comprehensive assessments of patients' physical capabilities, functional limitations, and therapeutic needs.
• Develop and implement customized care plans aimed at improving patient outcomes, incorporating feedback from physicians and interdisciplinary teams.
• Provide skilled physical therapy treatments, exercises, and interventions in home settings based on individual care goals established during assessments.
• Evaluate patient progress regularly, document findings accurately in electronic health records, and adjust therapy approaches as necessary to ensure effective rehabilitation.
• Educate patients and, where applicable, their family members or caregivers on treatment methods, exercises, and the use of medical appliances or devices to support treatment plans.
• Manage and prioritize your caseload efficiently, ensuring clarity in communication and timely turnover of patient assignments.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, including adults and children recovering from surgeries, injuries, or managing chronic conditions. Your expertise in physical therapy will directly contribute to their recovery journeys, helping patients regain independence and improve their functional mobility in their daily lives.

Qualifications

• Active and valid state licensure in Physical Therapy.
• Bachelor's degree in Physical Therapy; a Doctor of Physical Therapy (DPT) degree is preferred.
• Basic Cardiac Life Support (BCLS) certification must be obtained within 30 days of employment.
• No prior experience is required, making this an excellent opportunity for new graduates looking to start their careers.

Preferred Experience

• Experience in home health physical therapy or a clinical rotation focused on home health services.
• Familiarity with electronic health record systems and documentation requirements in a home care environment.
• Knowledge of therapies related to orthopedics, neurological rehabilitation, and geriatric care is advantageous.

Compensation and Benefits
