Pharmacist Inpatient

Job Summary:


The Pharmacist Inpatient is responsible for overseeing the medication use process within an outpatient infusion center, including order verification, compounding evaluation, and treatment plan assessment. This role requires expertise in infusion medications, including oncology and non-oncology therapies, patient education, and adverse reaction management. The pharmacist will ensure safe and effective medication therapy while collaborating with healthcare teams to optimize patient outcomes. A minimum of one year of experience with outpatient infusion is required.

Responsibilities:

• Float between units as assigned, primarily focusing on outpatient infusion services.
• Verify orders for non-hazardous and hazardous outpatient infusion medications, including oncology treatments.
• Evaluate and verify IV infusion compounding products for accuracy and safety.
• Assess oncology and non-oncology treatment plans for appropriateness and safety.
• Review and verify insurance coverage prior to initiating treatment plans.
• Provide patient education related to infusion medications and therapy.
• Participate in the management and response to infusion adverse reactions.
• Maintain compliance with hospital policies and regulatory requirements.

Required Skills & Certifications:

• At least 1 year of experience with outpatient infusion pharmacy practice.
• Licensure as a Pharmacist in the state of practice.
• Strong knowledge of oncology and non-oncology infusion medications.
• Proficiency in medication order verification and compounding evaluation.
• Ability to evaluate treatment plans and insurance coverage.
• Excellent communication and patient education skills.

Preferred Skills & Certifications:

• Experience in a Critical Access or Level III Trauma Center environment.
• Familiarity with Epic electronic medical record system.
• Experience in managing adverse reactions related to infusion therapy.

Special Considerations:

• No on-call responsibilities.
• No weekend work required.
• Block scheduling is required for shifts.
• Scrubs provided for OR, Critical Care, ED, and Maternal Child units; any scrub color allowed elsewhere.

Scheduling:

• Block schedules are mandatory, with no weekend or on-call shifts.
• Standard orientation hours reduced compared to typical 40-hour Non-Billable Orientation (NBO).
