At Good Samaritan Health Center, we are dedicated to providing quality healthcare services with a focus on holistic health and wellness outcomes, particularly for underserved communities. We are looking for a committed RN Case Manager to join our team and contribute to our mission of offering compassionate, comprehensive patient care. In this role, you will be responsible for assessing, planning, facilitating, and advocating for options and services through a continuum of care from point of contact through discharge on assigned cases. This position will have a significant impact on the quality of care offered and the ongoing success of our healthcare initiatives.
