Bilingual Community Health Worker — In-Home Visits (Travel)

VillageCare

VillageCare is seeking an enthusiastic Community Health Worker to serve as a liaison between VillageCareMAX members and the care team, including in-home visits to ensure appropriate care. The role requires bilingual ability, travel across the New York Tri-State Area, and collaboration with an interdisciplinary team. Candidates should have 3 years in health/social settings, a high school diploma (Associate's preferred), and strong organizational and communication skills. #J-18808-Ljbffr VillageCare
