Key Responsibilities\n

• Conduct comprehensive mental health assessments to identify patient needs while considering their physical health conditions.

• Facilitate individual and group therapy sessions, customizing approaches to each patient’s unique circumstances and therapeutic goals.

• Actively collaborate with medical providers to ensure coordinated care, discussing treatment progress, barriers, and patient outcomes during regular case conferences.

• Provide immediate crisis intervention and support to patients experiencing mental health emergencies, employing proven techniques to stabilize and promote safety.

• Educate clients on the relationship between mental and physical health, empowering them with strategies to manage their overall well-being effectively.

• Maintain meticulous and confidential documentation in accordance with legal, ethical, and organizational standards.

Patient Population / Client Focus\n

In this role, you will work with a diverse population of clients, many of whom present with co-occurring medical conditions alongside their mental health challenges. This includes individuals experiencing anxiety, depression, PTSD, and other mood disorders, often complicated by chronic physical health issues. Your role will not only address their mental health needs but also involve collaborating with various healthcare providers to ensure a seamless integration of care, enhancing patient stability and recovery.

Qualifications\n

• Master's degree or higher in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Active and unrestricted licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Psychoanalyst, Psychologist, or Licensed Marriage and Family Therapist (LMFT) in New York.

• Demonstrated experience in providing mental health services to clients with co-occurring medical conditions.

• Exceptional interpersonal and communication skills, with the ability to build rapport and trust with clients through virtual platforms.

• A strong ethical foundation, ensuring confidentiality and professionalism at all times.

• Proficiency in utilizing electronic health record (EHR) systems and other digital tools relevant to mental health practice.
