Key Responsibilities\n

• Conduct thorough psychosocial evaluations and assessments to identify the needs and preferences of clients and their families.

• Collaborate with interdisciplinary team members in the development and implementation of individual family service plans (IFSPs) tailored to the unique needs of hospice clients.

• Monitor and assess outcomes, adjusting care plans as necessary to ensure effectiveness and quality of service delivery.

• Provide expert counseling and emotional support to seniors and their families, addressing issues related to grief, transition, and end-of-life planning.

• Educate families about available resources and options to empower them in making informed decisions about care.

• Ensure compliance with all federal and state regulations and standards related to social work practice within a hospice setting.

• Build trusting relationships with clients, families, and team members to foster a supportive and constructive care environment.

Patient Population / Client Focus\n

This role focuses on working with seniors receiving hospice care, along with their families. Clients are often dealing with complex emotional challenges related to end-of-life issues, including grief, loss, and existential distress. Qualifications\n

• A Master’s degree in Social Work (MSW) or a related field.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Texas is required.

• Minimum of two years’ experience in a clinical social work role, preferably in hospice or geriatric settings.

• Strong understanding of assessment, diagnosis, and treatment methodologies applied to senior populations.

• Exceptional verbal and written communication skills, with the ability to articulate complex emotional issues clearly.

• Proficiency in organizational and planning skills to manage caseloads effectively.

• Bilingual proficiency in English and Spanish is preferred, enhancing communication with a diverse client base.

Compensation and Benefits\n
