RN Case Manager: Care Coordination & Discharge Planning

Swedish Medical Center in Colorado is seeking a Clinical Coord Case Manager to optimize patient care post-discharge and coordinate care with the multidisciplinary team. You will engage discharge planners, social workers, and UR RNs to define plans of care and ensure timely post-acute referrals. The role requires Colorado nursing licensure, BSN preferred, and 3–5 years of acute care experience, with supervisory experience valued. #J-18808-Ljbffr
