Job Description
Physical Therapist - Home Health

Join an outstanding team of healthcare professionals dedicated to providing quality care in a home health setting. As a Physical Therapist, you'll be responsible for planning and administering medically prescribed physical therapy treatments aimed at restoring function and relieving pain for patients with various injuries and disorders.

Key Responsibilities

Plan and implement individualized physical therapy programs based on patient assessments and treatment goals.
Provide education and information to patients and their caregivers regarding treatment plans and exercises.
Participate in discharge planning and collaborate with the healthcare team to optimize patient outcomes.
Complete all required documentation in compliance with policy and procedures.
Direct and support patient participation in selected tasks to enhance performance and recovery.

Schedule

This position offers a full-time schedule of Monday through Friday, from 8:00 AM to 5:00 PM, with weekends required for orthopedic cases every 4th or 5th weekend.

Qualifications

Minimum of 1 year of experience in physical therapy; first-time travelers are encouraged to apply.
Experience in home health settings is preferred.
Familiarity with MatrixCare or Epic is a plus, but not required.
Valid Physical Therapy license for the state of Virginia is required.
Current CPR certification is required.

Additional Information

This role involves an average of 6 to 7 weighted visits per day and reimbursement for mileage at the IRS rate. Join us in delivering compassionate care that makes a difference in the lives of our patients!
