About the Job Registered Nurse (RN) - Home Health & Hospice. Blize Healthcare | Bay Area, CA | Flexible Territory. Blize Healthcare is seeking a compassionate and dependable Registered Nurse (RN) to join our growing home health & hospice team. In this role, you will deliver high-quality, patient-centered care in the comfort of patients' homes and care facilities across the Bay Area. This is an excellent opportunity for a nurse who values independence, flexibility, and meaningful patient relationships . What You'll Do:
- Provide skilled nursing care in accordance with physician orders and care plans.
- Monitor patient conditions by taking vital signs, assessing symptoms, and documenting changes.
- Administer medications and treatments, including wound care.
- Perform physical assessments and basic diagnostic procedures.
- Collaborate with physicians and care teams to adjust treatment plans as needed.
- Educate patients and families on care plans and health management.
- Ensure continuity of care during after-hours and weekend coverage.
- Active Registered Nurse (RN) license in California.
- Minimum 1 year of nursing experience.
- Home Health or Hospice experience strongly preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
- Physical ability to lift up to 50 lbs when necessary.
- Valid BLS Certification.
- Valid California Driver's License and reliable transportation.
- Start of Care (SOC): $180.
- Recertification: $1300.
- Resumption of Care: $1400.
- Discharge/Death Visit: $120.
- PRN Progress Visit: $80.
- Hourly Rate: $48.
- Mileage reimbursement.
- Flexible scheduling.
- Supportive clinical team environment.
