Case Manager IR

IR Case Manager will assist clients at all phases of their treatment program. They will monitor client behaviors, assist clients in making calls and scheduling appointments, work with the treatment team to develop discharge plans for each client, including transfer to TL, referrals, medical and mental health follow‑ups, and present educational material to clients. Roles and Responsibilities Participate and interact in daily activities of the clients, develop relationships and trust, and maintain Samaritan standards during unstructured time periods. Assist in the development of the service plan for each client, motivate committed treatment, support transition, and foster independence for lasting community involvement. Assist the treatment team with aftercare referrals, including outpatient mental health programs, intensive outpatient programs, and half‑way houses. Discuss weekly with the Clinical Director and appropriate referrals to SRC’s TL program. Coordinate with clients who have legal concerns, including those on state probation. Send weekly progress reports to Forensic Social Workers. Send admission/discharge faxes to concerned parties within the legal system. Responsible for occasional transport of clients to community appointments in the area. Connect with housing administrations, childcare facilities, and advocacy groups. Participate in the IR Lecture/Seminar rotation. Communicate effectively with Admissions Clerks, Counselors, and Nursing staff. Utilize community resources to aid clients in obtaining driver’s license, social security card, birth certificate, food stamps, TNCare, grants for sober living/housing, etc. Document activities in approved systems—progress of each client’s participation is a key element to this position. Other related duties as assigned. Requirements High School Diploma; some college preferred. Driver’s License with an “F” endorsement. Understanding of recovery process and treatment. Ability to work independently and with a team. Physical Demands / Work Environment Must be able to sit and stand for 8 hours. Must be able to walk or stand for up to 50% of the shift.
