Key Responsibilities\n

• Conduct comprehensive assessments to identify the social, emotional, and psychological needs of clients through telehealth platforms.

• Provide empathetic counseling and support to individuals and families facing a range of challenges, focusing on their unique circumstances and needs.

• Utilize evidence-based therapeutic approaches for individual and group counseling sessions, ensuring that methodologies align with the latest research and best practices.

• Develop and implement personalized service plans that address clients’ specific goals, facilitating their journey toward recovery and wellbeing.

• Advocate for clients’ rights and access to necessary resources, addressing relevant social justice issues and facilitating connections to community supports.

• Contribute to program development and evaluation, participating in grant reporting to measure and enhance service effectiveness.

• Collaborate with community organizations, government agencies, and other professionals to streamline efforts and enhance service offerings.

Patient Population / Client Focus\n

In this role, you will primarily serve clients connected to our hospice center, addressing the profound challenges faced by individuals at the end of life and their families. This includes managing grief and loss, providing emotional support, and connecting clients to necessary resources to enhance their quality of living during this sensitive time. Your expertise in trauma-informed care will be essential in creating a safe space for individuals to express and process their experiences.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW).

• Strong communication skills, demonstrating empathy and understanding toward clients' unique needs and situations.

Preferred Experience\n

Compensation and Benefits\n
