Key Responsibilities\n

• Perform thorough intake and psychosocial assessments to understand patients' needs and develop personalized care plans.

• Collaborate with our Chemical Dependency Recovery Program (CDRP) team to devise, present, and refine treatment plans, ensuring alignment with best practices in addiction therapy.
\n
• Participate in clinical case conferences, contributing insights and recommendations to enhance patient care pathways.
\n
• Conduct educational lectures focused on chemical dependency, equipping patients and their families with vital information about addiction and recovery processes.
\n
• Deliver targeted individual and group counseling services for chemical dependency, offering empathetic support tailored to each client's circumstances.
\n
• Provide specialized therapy sessions, including adolescent therapy, familial counseling, and multi-family therapy, as assigned, to facilitate connection and open communication among family members.

• Offer comprehensive case management services, which encompass resource and referral coordination to assist patients in accessing necessary community supports.

Patient Population / Client Focus\n

Your work will primarily focus on patients receiving hospice care, as well as their families who are navigating the complex emotional landscape surrounding end-of-life issues and chemical dependency. Providing support to this vulnerable population requires a deep understanding of grief, loss, and the implications of addiction, making flexibility, empathy, and ethical sensitivity paramount in this role.

Qualifications\n

• A Master’s degree in Social Work (MSW) or a related field is required.

• Licensure/Certification/Registration: Valid LICSW credential in the state of Washington.

• Demonstrated experience with a minimum of two (2) years post-graduate work in supervised addiction medicine therapy within the last five (5) years.

Preferred Experience\n

While not mandatory, candidates with experience in hospice care, palliative counseling, or educational contributions to addiction-related programs may have an advantage. Familiarity with community resources and experience collaborating with interdisciplinary teams will also be beneficial.

Compensation and Benefits\n
