Job Summary:

We are seeking a compassionate and experienced Social Services Director to lead our Social Work Department at the Coastal Health & Rehabilitation Center, a renowned provider of skilled nursing and rehabilitation services in Daytona Beach, Florida. As a key member of our leadership team, you will play a vital role in ensuring the well-being and quality of life for our residents and their families.

Responsibilities:

• Lead social services for residents and families, developing social histories, assessments, and care plans
• Participate in interdisciplinary care meetings to coordinate resident care
• Support discharge planning and placement to ensure a seamless transition for residents
• Ensure compliance with state and federal regulations and guidelines
• Collaborate with healthcare professionals to provide person-centered care

Requirements:

• Bachelor's degree in Social Work or a related field (Master's preferred)
• Strong interpersonal and leadership skills
• Experience in long-term care or healthcare social services
• Knowledge of state and federal social services guidelines
