Pair Team is seeking a full-time Lead Care Manager to join our distributed care team serving California communities. This role focuses on engaging individuals with Serious Mental Illness, Substance Use Disorder, homelessness, and high medical needs, through empathetic outreach and coordinated care. The position largely works from home (about 90%), with 2–3 on-site community visits monthly near your city. Bilingual in English/Spanish is required; in California, you will collaborate with our #J-18808-Ljbffr Pairteam
