Medical Services CoordinatorNAFI Connecticut provides programs, consultation, and support services in a variety of settings across Connecticut, New York and Rhode Island. We believe that people can change, and we work to empower each individual in their own care.We are currently seeking an organized, detail-oriented, and collaborative Medical Services Coordinator to support the medical care of adolescents residing in our 16-bed residential program in Litchfield, Connecticut. Working closely with the Program Nurse and residential leadership team, the Medical Services Coordinator serves as a point of coordination for residents' medical services. This role is responsible for scheduling and tracking medical appointments, coordinating communication with healthcare providers, managing insurance and billing issues, identifying community providers to meet the diverse healthcare needs of youth, and ensuring all medical documentation is completed accurately and maintained in electronic health records and required external portals. The ideal candidate thrives in a fast-paced, dynamic environment, is highly organized, and can effectively respond to evolving requirements from medical providers, funders, regulatory bodies, and other stakeholders. This position plays a critical role in ensuring adolescents receive timely, coordinated, and comprehensive healthcare services. Strong English verbal and written communication skills required.Responsibilities Include:Work in close collaboration with the Program Nurse to ensure continuity of care and smooth coordination of all medical servicesCoordinate all medical, dental, vision, specialty care, and other healthcare appointments for youth residing in the programServe as a liaison with physicians, specialists, hospitals, pharmacies, insurance companies, and other healthcare providersCoordinate referrals, follow-up appointments, diagnostic testing, and specialty care servicesServe as the point person for insurance authorizations, billing questions, claims issues, and healthcare coverage concernsEstablish and sustain referral networks with licensed community medical, behavioral health, dental, and specialty providers to ensure youth receive coordinated, comprehensive, and holistic care that is responsive to their unique needs and promotes safety, permanency, well-being, and successful outcomesEnsure all medical documentation is completed accurately and uploaded into the agency's electronic health record and required external portals within established timelinesHelp maintain organized medical records and tracking systems that support regulatory compliance and quality careCollaborate with residential staff to coordinate transportation and preparation for appointments.
