Care Coordinator

Job Title Care Coordinator Responsibilities Build and maintain trusted relationships with patients, prescribers, and client stakeholders through proactive communication and timely execution of deliverables. Perform program welcome calls and post-benefit investigation calls, explaining coverage options to patients and physicians. Manage client inquiries that cannot be resolved by the client and handle HCP inquiries according to business rules. Handle inbound calls following program‑approved FAQs, triaging patients to internal or external resources as appropriate. Provide personalized case management, including outbound communication to HCPs and patients about benefit coverage and next steps. Conduct benefit investigations, prior authorizations, denials appeals, tier exclusions and formulary reviews using electronic tools and manual processes. Coordinate nurse teach‑ins with field‑based nurse educators and support adherence services through nurse follow‑up when applicable. Identify and connect patients with peer support resources. Coordinate shipment of products through the patient assistance program or bridge program from the Valeris pharmacy. Proactively manage reverification of prior authorizations or re‑enrollment for patient assistance programs. Report adverse events, product complaints, special‑situation reports and medical inquiries per SOPs and business rules. Document all activities in the Valeris Lynk system in accordance with business requirements. Ensure consistent adherence to SOPs and maintain high quality of service. Attend training sessions and meet corporate policy and procedure deadlines. Qualifications Bachelor’s degree preferred; customer‑service or healthcare industry experience is a plus. Ability to work an 8.5‑hour shift between 8a.m. and 9p.m., Monday–Friday. Bilingual in English and Spanish, or Chinese/Mandarin, is an advantage. Must be able to sit for extended periods; minimal travel flexibility preferred. Typing speed of at least 35 words per minute with 97% accuracy.
