Work Environment

As a Physical Therapist at Strengthen Physical Therapy, Inc., you will work in the comfort of patients' homes, which allows for a unique one-on-one therapy experience. Key Responsibilities

• Conduct comprehensive assessments and evaluations of each patient's physical capabilities, challenges, and specific therapy needs.
• Design, implement, and oversee personalized treatment plans that focus on enhancing strength, improving mobility, and achieving functional independence.
• Utilize evidence-based therapeutic modalities including therapeutic exercises, manual therapy, and assistive devices to alleviate physical impairments.
• Regularly monitor and document each patient's progress, making adjustments to treatment plans as necessary based on individual responses.
• Engage actively with an interdisciplinary team, collaborating closely with physicians, nurses, and occupational therapists to provide well-rounded care.
• Educate patients and their families on best practices for home exercise programs, injury prevention strategies, and techniques to promote long-term physical health.
• Ensure compliance with all relevant state and federal regulations regarding home health care services and maintain adherence to professional standards.

Patient Population / Client Focus

The primary focus of this role is on geriatric patients, who often have a range of complex health issues impacting their physical abilities. You will develop strong relationships with each patient and their families, providing compassionate care that is sensitive to their needs and preferences. Your work will be crucial in helping them regain independence and improve their quality of life in the comfort of their own homes.

Qualifications

• A current and valid Physical Therapist license in the state of New York.
• A Bachelor's Degree in Physical Therapy is required, with a Master's Degree or Doctorate in Physical Therapy being preferred.
• Demonstrated knowledge and practice in physical therapy methodologies and therapeutic interventions.
• Proficiency in electronic health record (EHR) systems for documentation and reporting purposes.

Preferred Experience

• A minimum of 2 years of experience as a Physical Therapist, preferably in a home health care or skilled nursing facility setting.

Compensation and Benefits

The salary range for this position is highly competitive, offering between USD 90,000 to 110,000 annually, complemented by a comprehensive benefits package.
