Position Overview

Horizon Flex Therapy is seeking a dedicated full-time Physical Therapist to join our team in Jacksonville, Florida. This role is critical in partnering with patients on their healing journey, focusing on restoring motion and enhancing their quality of life through personalized care plans. Work Environment

This position requires onsite service delivery within the home care setting, offering you the unique opportunity to build relationships with patients in their own environments. By working directly in the homes of your clients, you will be able to observe and adapt therapies based on their unique living situations, ensuring a comprehensive approach to rehabilitation.

Key Responsibilities

• Collaborate with patients to identify their individual goals and develop personalized treatment plans focused on rehabilitation and recovery.
• Responsible for managing a caseload of approximately 2-3 patients per hour, ensuring quality interaction and care.
• Implement effective physical therapy interventions aimed at improving patient mobility and function, while reducing dependency on medications or surgical procedures.
• Design and deliver fitness- and wellness-based programs tailored specifically to meet the physical needs of each patient.
• Document patient evaluations, progress notes, and treatment plans using EMR software such as WebPT, maintaining accurate and comprehensive records.
• Communicate effectively with patients, motivating them through their treatment process to achieve optimal functions.
• Consult with other healthcare professionals to provide integrated care and share insights regarding patient treatments.
• Evaluate patient outcomes regularly and modify care plans as necessary to ensure the utmost effectiveness in rehabilitation.

Patient Population / Client Focus

You will primarily work with adults recovering from orthopedic injuries, surgeries, or managing chronic conditions. With a focus on functional rehabilitation, your role will enable patients to regain mobility and independence, enhancing their overall quality of life.

Qualifications

• Bachelor's degree in Physical Therapy or higher, with preference for a Master of Physical Therapy (MPT) or Doctor of Physical Therapy (DPT).
• Current Florida Physical Therapist License, with no disciplinary actions on record.
• National Provider Identifier (NPI) number or the ability to obtain one prior to starting employment.
• Proven expertise with manual therapy techniques and exercise prescription.

Preferred Experience

• Prior experience in an orthopedic outpatient setting, with a strong understanding of the principles behind rehabilitation therapies.
• Comfortable working independently and collaboratively within a multidisciplinary team.
• Proficiency in EMR systems, specifically WebPT, to streamline documentation and patient management.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 120,000 to 130,000, commensurate with experience. Additionally, our comprehensive benefits package includes:
