Key Responsibilities\n

As an LCSW, you will engage in a variety of essential tasks aimed at promoting effective treatment and support. Your responsibilities will include:

• Utilizing the DSM (Diagnostic and Statistical Manual) criteria for diagnosing substance abuse and determining the appropriate levels of care based on ASAM (American Society of Addiction Medicine) standards.

• Developing and documenting tailored treatment plans, focusing specifically on substance abuse counseling, and ensuring the inclusion of clients’ unique circumstances and strengths.

• Creating discharge and continuity of care plans to facilitate clients' transition to higher or lower levels of care as necessary.

• Maintaining accurate and thorough documentation of all counseling sessions, assessments, and follow-ups in clients’ clinical records.
\n
• Managing an active caseload, which includes case management, treatment planning, and utilization review updates to support comprehensive care.
\n
• Coordinating with other service providers and community resources, including legal, educational, and employment agencies, to ensure holistic support for clients and their families.

• Providing crisis intervention by applying a multidisciplinary team approach, facilitating immediate support for clients experiencing acute issues.

You will primarily work with clients facing challenges associated with substance use disorders and complex grief, particularly those at the hospice stage. Your efforts will focus on delivering empathetic and culturally competent care to individuals navigating difficult life transitions. Qualifications\n

The ideal candidate must possess the following qualifications:
