Work Environment

This position takes place primarily within patients' homes, allowing for a personalized approach to therapeutic care. Key Responsibilities

• Conduct comprehensive assessments to evaluate patients referred by physicians and develop tailored treatment plans.
• Implement therapeutic interventions based on the latest clinical guidelines, addressing orthopedic, pediatric, neurologic, and sports medicine cases.
• Deliver skilled physical therapy services in a home care setting, following physicians' care plans to produce measurable improvements in patients' health.
• Educate patients and their families on rehabilitation goals, home exercises, and strategies to manage and enhance physical well-being.
• Maintain meticulous documentation of patient progress and treatment efficacy while meeting utilization goals and business objectives.
• Exemplify excellent customer service and professionalism in every patient interaction, contributing to overall team effectiveness.

Patient Population / Client Focus

In this role, you will serve a diverse clientele, focusing particularly on orthopedic patients experiencing a range of conditions from post-surgical recovery to chronic musculoskeletal issues. The need for targeted rehabilitation throughout various stages of healing means you will encounter both children and adults with unique therapeutic requirements, enhancing your professional expertise.

Qualifications

• Bachelor's degree in Physical Therapy or equivalent from an APTA or CAPTE-accredited institution.
• A valid state license to practice physical therapy in Florida.
• Basic Life Support (BLS) certification from the American Heart Association.
• Proficiency with technology including electronic medical records (EMR) systems and general computer skills.
• Exceptional written and verbal communication abilities to effectively engage with patients and collaborate with team members.

Preferred Experience

Candidates with previous experience in home health care settings or specialized pediatric and orthopedic therapy will be prioritized. A proven track record of developing patient-centered care plans and achieving strong outcomes will be viewed as advantageous.
