Registered Nurse – Home Health

Shift: 7:00 AM - 7:00 PM

Contract Weeks: 13 Guaranteed Hours: 36.00

Requirements:

• Bachelor's Degree in Nursing (BSN): Preferred
• Registered Nurse (RN) licensure in the state of practice: Required
• Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from an approved vendor: Required
• Valid Driver's License (DL) and must be at least 21 years of age or older: Required
• Current automobile insurance and a reliable automobile: Required
• At least two years recent experience in the field: Required

Roles & Responsibilities:

• Plans, organizes, and directs home care services to meet the needs of patients in their homes.
• Manages a patient caseload effectively and efficiently.
• Provides patient care independently while collaborating with other core disciplines according to the patient's plan of care.
• Completes initial and ongoing comprehensive assessments of patients' needs and implements care plans accordingly.
• Counsels and educates patients and families on healthcare needs and nursing care.
• Accurately documents all relevant patient information in accordance with established policies and procedures.
