Key Responsibilities

• Deliver customized physical therapy interventions aimed at alleviating pain, restoring function, and promoting overall health.
• Assess clients' physical therapy needs through comprehensive evaluations, including diagnostic tests related to muscle, nerve, and joint function.
• Develop and implement individualized care plans in collaboration with clients, physicians, and interdisciplinary teams.
• Conduct home evaluations to tailor therapy to the client's living conditions, enhancing their independence and quality of life.
• Administer hands-on therapy techniques including therapeutic exercises, massage, and the application of physical agents as per physician prescriptions.
• Guide and mentor Physical Therapy Assistants in patient care activities while ensuring they adhere to the established care plans and practices.
• Document clinical records, including evaluations, progress notes, and discharge plans, ensuring compliance with local, state, and federal regulations.
• Engage in case conferences and interdisciplinary communication to evaluate and refine the total treatment approach.
• Provide education to clients and their families about home therapy practices and equipment effectively.
• Maintain timely documentation and reporting of patient progress in line with organizational policies.

Compensation and Benefits
