RN Care Manager – Child Center – Long Term Care Providence Health and Services, Portland, OR. Full‑time position (Monday‑Friday, 7 a.m.–4 p.m.). Description The RN Care Manager will lead nursing care for residents in the Long‑Term Care unit of the Child Center, which has 46 beds and serves both pediatric and adult patients with complex medical and developmental conditions. Responsibilities

• Manage nursing care of assigned residents, ensuring maximum independence and self‑direction.
• Develop, implement, and document individualized care plans in accordance with the nursing process and the Nurse Practice Act.
• Coordinate care provision with physicians and interdisciplinary team members in a collaborative relationship.
• Ensure care plans meet residents’ individualized needs and reflect professional development within nursing practice.
