Trauma‑Informed Clinical Social Worker (Discharge Focus)

You will provide crisis intervention, coordinate care, and work collaboratively within a multidisciplinary team to ensure timely and safe discharges while addressing continuing care needs. This role requires a Master's degree from a CSWE accredited school of social work and a California state license. Experience in healthcare and trauma-informed care is essential.
