Field-Based RN Case Manager — Home Visits & Travel

Optum Home & Community Care in Georgia is seeking a Full-time Float Registered Nurse Case Manager to support members in home settings. You will travel across the state, perform in-home visits, telephonic outreach, and collaborate with an interdisciplinary care team to coordinate care and ensure outcomes. The role requires an RN license, at least 2 years of nursing experience, and the ability to work with diverse teams.
