Job Responsibilities

• Conduct thorough evaluations of patients in both inpatient and outpatient settings and craft personalized treatment plans
• Implement physical therapy interventions aimed at improving mobility, strength, stability, and overall function
• Partner with nurses, physicians, case managers, and a range of rehabilitation professionals to enhance patient outcomes
• Provide education to patients and their families regarding therapeutic goals, discharge planning, and necessary home exercises
• Maintain accurate documentation of patient evaluations, treatments, and progress in line with facility standards
• Engage in interdisciplinary team rounds and conferences to foster coordinated care

Qualifications

• Doctorate in Physical Therapy (DPT) or an equivalent accredited degree

Licensure and Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred but not obligatory
• Specialized certifications such as neurological or geriatric therapy are highly valued

Key Skills

• Strongly prefer candidates with relevant work experience
• Ability to effectively treat patients dealing with complicated medical and mobility challenges
