Job Responsibilities

• Conduct evaluations for patients in both inpatient and outpatient environments and craft personalized treatment plans
• Implement physical therapy interventions directed at enhancing mobility, strength, balance, and overall function
• Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to maximize outcomes
• Inform and educate patients and families regarding therapeutic objectives, discharge planning, and home exercise regimens
• Maintain documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards
• Engage in care team rounds and interdisciplinary meetings to ensure comprehensive care delivery

Qualifications

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution
• State licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (e.g., neurological, geriatric) are beneficial but not mandatory

Skills

• Experience preferred but not necessary
• Ability to treat patients facing complex medical and mobility challenges
• Strong communication and teamwork abilities
• Familiarity with digital charting and clinical documentation systems
