Description

• Conduct patient evaluations in both inpatient and outpatient settings and create custom treatment plans
• Deliver physical therapy interventions aimed at improving mobility, strength, balance, and overall function
• Work in tandem with nurses, physicians, case managers, and other rehabilitation experts to achieve optimal results
• Provide education to patients and their families on therapeutic objectives, discharge planning, and home exercise routines
• Maintain thorough documentation of evaluations, treatments, and progress as per facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to assure synchronized care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• Current CPR certification is preferred
• Specialized certifications (e.g., neurological, geriatric) are advantageous, but not mandatory

Skills

• Previous clinical experience strongly preferred
• Ability to manage complex medical conditions and mobility challenges
