Job Description

• Conduct evaluations of patients in inpatient or outpatient settings and craft personalized treatment plans
• Implement physical therapy strategies focused on enhancing mobility, strength, balance, and overall function
• Work closely with nurses, doctors, case managers, and rehabilitation specialists to achieve optimal patient outcomes
• Inform patients and their families about therapeutic objectives, discharge processes, and home exercise routines
• Maintain thorough documentation of evaluations, treatment sessions, and patient progress in accordance with all regulatory and facility protocols
• Engage in multidisciplinary team rounds and conferences to guarantee seamless care delivery

Education

• Doctorate in Physical Therapy (DPT) or a similar degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
