Key Responsibilities\n

• Conduct thorough, culturally sensitive biopsychosocial assessments that encompass clients' mental health, substance use, and medical needs in alignment with our primary care integration framework.

• Deliver evidence-based behavioral health services to individuals, couples, and families, encompassing counseling, intake assessments, treatment planning, crisis intervention, and referrals for substance use disorder treatments.
\n
• Document all interactions and assessments electronically, including progress notes and suicide risk assessments, maintaining compliance with healthcare regulations and standards.
\n
• Act as a liaison to connect clients with essential community resources, medication assistance programs, and social work supports to enhance the overall treatment experience.

• Collaborate with Quality Improvement teams to streamline processes for data collection, ensuring that treatment outcomes are consistently evaluated and improved.

Patient Population / Client Focus\n

This position focuses primarily on serving adults undergoing detoxification and those at risk for substance abuse and mental health crises. We cater to a diverse clientele, acknowledging the impact of cultural and social factors on each individual's care journey. Qualifications\n

• Current and valid license as a Licensed Clinical Social Worker (LCSW) in the state of New York.

• A Master’s degree in Social Work with a minimum of three years of post-licensure experience in a clinical setting.

• Ability to exercise independent judgment and provide essential direct services to clients effectively.

Compensation and Benefits\n
